At Kindfulness Counseling, I believe clarity and transparency are essential to creating a safe, supportive space—for both therapy clients and supervisees.
Below you’ll find an overview of my practice policies, communication boundaries, and what to expect when working together.

These policies are designed to help both of us maintain consistency, trust, and safety—whether we’re meeting in person or virtually.

All required intake and consent forms for therapy clients are completed securely through Headway, my electronic health record (EHR) and billing platform.
You do not need to print or sign these versions—Headway will prompt you to complete them electronically before your first session.

A few additional forms are available below for download when needed. These are used for specific purposes, such as coordination of care, supervision payments, or formal supervision agreements.

This form authorizes me to coordinate care with other professionals—such as psychiatrists, primary care doctors, or schools—when you choose to provide written consent.

Outlines your rights as a supervisee, including the right to fair and ethical treatment, clarity in expectations, feedback, confidentiality, and professional respect throughout the supervision process. Details the structure and expectations of our supervision relationship, including scheduling, payment, recordkeeping, confidentiality, and ethical responsibilities under Maryland and Delaware licensing standards.
